COVID-19 SCREENING, GUIDELINES, WAIVER & POLICIES

POLICY

Purpose: Based on the US Center for Disease Control Guidelines, service providers, daily, are encouraged to screen all clients for signs of respiratory illness accompanied by fever.

Instructions: All clients entering The American Shave Classic Barber, LLC building must be screened prior and upon arrival. American Shave will maintain this record for 14 days from completion of this form and have this form available upon request from the Public Health Department.

If you answered NO to all questions you will be allowed entry to the building. Please be aware of the following protocols:

  • Respecting the shop’s sanitation and hygiene standards and processes posted within the shop.

  • Refraining from visiting the shop if you have a fever or communicable illness.

  • Refraining from visiting the shop if you are under an isolation or quarantine order/directive.

  • Curbside waiting room.

  • No walk-ins or online bookings.

  • By Appointments only until further notice.

  • Only clients receiving services will be allowed in the shop, unless you are accompanying a minor.

  • If you are accompanying a minor, maintain social distancing at all times.

  • You will receive a text when the barber is ready to service you.

  • Wear a face covering or mask at all times.

  • Children under 2 years old are not required to wear a face covering or mask.

  • Answer screening questions before and during your visit.

  • Must complete online waivers and forms prior to your appointment.

  • A no-contact forehead temperature check will be administered upon check in.

  • Washing hands for a minimum of 20 seconds or sanitize prior to beginning each service.

  • No waxing, beard or shave services will be rendered until further notice.

  • No Show policy is in full effect.

  • Please call 24 hours prior to canceling/rescheduling appointments.

  • NO exceptions will be made.
     

  • By accepting the form below I (client) am acknowledging the potential risk to contract the COVID-19 disease during services provided today and voluntarily agreed to accept services. I (client) further agree and hereby release The American Shave Classic Barber, LLC and its employees from any and all liability associated with your potential risk to contract NOVEL CORONAVIRUS (COVID-19).
     

  • * The person answering YES to any of the above questions is responsible for following-up with their primary care physician if needed.
     

  • By checking this box, I pledge to provide only correct and truthful information when completing this screening.

Do you have any of the following respiratory symptoms?
Have you had a (temperature 100.4*F or greater within the last 14 days)?
Are you feeling feverish?
Are you having chills?
Have you been in a facility or home with confirmed COVID-19 by lab test within the last 14 days?
Have you been with persons with confirmed COVID-19 by lab test within the last 14 days?

If YES to any, please call and cancel your appointment immediately.

CANCELLATION POLICY

Please allow 24 hours for appointment cancellations and a courtesy call in advance if you are running behind.  A 75% non-refundable charge may apply for "No-Show" clients. Any appointment not cancelled at a minimum of 1 hour in advance notice are also subject to 75% non-refundable charge.  Appointments who are more than 10 minutes late, are subjected to a shortened service time or may be denied services and subject to a 75% service charge.  Remember, your time as well as ours is valuable. No refunds or returns on services.  We hope to serve you soon!
